Membership Forms

Thank you for joining Kolot Chayeinu / Voices of Our Lives! Our congregational/membership year goes from Sept-Aug. This form contains the following five sections to be filled out: 

1 - Member/Family contact information
2 - Fair Share Dues Pledge
3 - High Holy Days Pledge
4 - CLP School Registration Forms
5 - Voluntary Participation Form

1) MEMBERSHIP FORM:

New members who fill out the form will have an account created for you after filling out your form.  You will receive an email to sign-in after it's created. 

Renewing members can skip filling out certain sections by logging in prior to filling out the form.  In order to not duplicate your account, please be sure to sign in prior to filling out the forms.

A portion of your dues must be received by December 31. Your total membership balance must be completed by June 30.  Membership pledges are 100% tax-deductible and are not refundable.

2) CLP FORM REQUIREMENTS:

Membership to Kolot Chayeinu / Voices of Our Lives is required of at least one parent / family member enroll in our CLP Program.  Families registering for only TorahTots (formerly StorahSteps) or K'tanim, are encouraged but not required to become members.  

CLP: We request full payment, or scheduling of payment, at time of enrollment.  If that is not possible, we require at least 25% of tuition at enrollment with the total balance by March 31.  Unlike membership, no portion of CLP Tuition is tax-deductible and CLP's refund policy is as follows:

Cancellation prior to first day of classes: You will be fully refunded minus a $50 administration fee. 
Withdrawal from classes during the first month: You will be fully refunded minus a $100 administration fee. 
Withdrawal from classes after first month: Refund will be prorated for time attended minus a $100 administrative fee. 
No refunds for withdrawal after December 31: And all outstanding fees are required to be paid in full by March 31.

3) PAYMENT INFORMATION:

Preferred payment is to remit your full balance at the time of renewal/enrollment. Through our online payment portal, you can use your credit card to: 1) pay in full, 2) schedule automatic payments at a flexible schedule that works for you, and 3) LOGGED IN MEMBERS: can choose to "Bill My Account" and create charges without payment needed at this time.

PRIVACY POLICY: 

The personal and financial information Kolot Chayeinu members share with the congregation is handled with the utmost care.  Only a very small number of Kolot staff and Board members have access to this information and we maintain confidentiality as part of our responsibility for fiscal oversight, financial planning, and fundraising for the congregation.

Financial information is never made public except by special arrangement and consent.  If you have any questions or concerns, please contact the Rabbi, President of the Congregation, or Executive Director.
